All WISD students need:

• Utility bill or proof of residence in their school zone
• Payment for all bills owed from the previous school year (textbooks, library fines)
• Immunization records

New students additionally need:

• Birth certificate and social security card
• Legal papers (divorce/separation)

IMMUNIZATIONS:

• Diphtheria, Tetanus and Pertussis - Five doses; however, 4 doses meet the requirement if the 4th dose was given on or after the child's 4th birthday
  
  
• Polio - Four doses; however, 3 doses meet the requirement if the 3rd dose was given on or after the child's 4th birthday
  
  
• Measles, Mumps and Rubella - Two doses; both on or after the child's 1st birthday (Serologic evidence of immunity or infection is acceptable in lieu of vaccine)
  
  
• Hepatitis B - Three doses (Serologic evidence of immunity or infection is acceptable in lieu of vaccine)
  
  
• Varicella - One dose on or after the child's 1st birthday; Two doses if the vaccine was given at 13 years of age or older (Serologic evidence of immunity or infection is acceptable in lieu of vaccine)
